Please complete the following task to finalize the registration process:

1. Complete the online registration form found here (online registration form)
2. Submit a copy of your child’s updated immunization information (Immunization Requirements)
3. Submit a copy of your child’s birth certificate (new students only)
4. Schedule a time with our school office to sign the Private School Affidavit Form (this only need to be done once after the child turns 6 years old)
5. Print and complete the Record Release Form (this form is only required if the child is entering grades 1-8 and attended a school other than Grace the previous school year)
